The below forms are required after you have received written notification that your Substitute Application has been selected for consideration. If you have not applied for a substitute position yet, you can do so here.
(DO NOT SEND FORMS UNLESS CONTACTED BY HUMAN RESOURCES SUBFINDER OFFICE.)

Forms required for substitute teaching and/or support positions

Substitute teaching in the classroom REQUIRES one of the following to be sent with required forms:

  • College transcript showing total credit hours taken (minimum of 60 hours –
    they do not have to be official)
  • College transcript/diploma showing conferred date (they do not have to be official)
  • A letter from the college stating requirements have been completed for degree and date complete
  • If applicable, a statement of eligibility or copy of teaching license (expired or current)
